Loft Between 2 Tubes of Different Diameters

I am trying to loft between these two cylinders and have tried pretty much everything I can think of to get it to work, but nothing seems to work. I even tried redrawing them in a new part file, and it still does not work. Am I missing something? Thanks

When I try to make the loft, the preview shows up just fine, except when I try to complete it, it says "Create loft feature failed"

I figured it out. In order to make the loft, I couldn't have the solid already created. I needed to make an individual sketch for each circle and then loft the outer circles to make the initial shape and then loft the inner circles to cut the hole through the middle.

There is a better way.

Do you want a smooth flowing transition or do you want more of a "conical" transition as illustrated by @CCarreiras ?

The conical transition works fine, but just so I know in the future, how would I be able to have a smoother transition instead? Thanks again
