I am creating a drawing form template in Inventor 2010. I placed sketch on the form. But when I use the form, the sketch does not display. Is there a setting or something I need to do special to make the sketch show up?

Sketches have not "stuck" on templates, just like views. They may be there while you are editing the file but go away when you use it as a template.

Is this part of the border or the title block? (Did you look at "Customize drawing templates" in help?)

I forget which version it started but you can now have sketched symbols placed when you use a template, maybe you could make your sketch a sketched symbol.
