Why does my bolted connection always ends up like this

Can you upload your part and assembly file?

It appears that the termination face has been set incorrectly - the current bolted connection window doesn't match what has already been placed.

Thank you for the reply, the issue is now solved, i clicked the wrong "click to add fastener" button which made inventor to put a washers over the unintended face.
