Post processor outputting Z rapid before XY position when using 4th axis

This is the ProtoTrak GCD Milling post processor.  I believe it is base on the Fanuc -Inverse Time and A-axis.  The first toolpath is output with XY position first, then Z rapid to clearance plane. Subsequent operations are being generated with Z rapid to clearance, then XY position.  This is especially troublesome because the Trak machine moves to current WCS X0Y0 at every operation when 4th axis is used. I attached a sample setup that demonstrates the issue. When posting this with the Fanuc -Inverse Time and A axis, Haas or Tormach post processor, all operations output XY position then Z rapid.  I can't figure out what part of the post processor is causing the incorrect order of operations.
