Toolpath doesn't match simulation

I've got a simple turned part with an external thread and i've noticed that the toolpath generated does not match the simulation or what is actually machined.

I've had to extend the back side offset well further than it should be to get it to be the correct length. See the screenshots for details. I've tried clearing and re-generating the toolpath. Any ideas?
