Internal groove not working

I can not seem to get this internal groove to fully work. Please help.

With a ball grooving tool you should define the cutting point on center. In the tool library select the tool, select Setup, Compensation, Insert Center. You should also select your lead in and out geometries so the tool can bend the radii.

What is wrong with the toolpath in the file you attached? Did not look too bad to me.  I always run full radius cutter to tip tangent since that is where I actually touch them off vs shifting them to the center of the cutter.
