Tapered angle in sheet metal

Hello, 

I am struggling with how to model this piece (see photo) in Fusion's sheet metal function. Any ideas? The tapered angle is giving me some troubles.

Here's one way to get the shape. 

I think with careful measurement, you should be able to get quite close to the actual piece.  If the top and bottom edges of the bent portion are perpendicular to the flats, then I think they were machined after bending.

This is how I was attempting to do it, which totally works! But, the answer below yours does it using patch initially which makes for a cleaner, quicker to build model. Thank you for helping out!
