Sweep, odd surface result...

I've had this issue every once and awhile and I can work around it, but I thought I might as well post this and see if I can figure out what's going on. The issue can be seen in this screencast:

 

 

The preview looks exactly like what I'd expect, but then once I click ok, the resulting surface is different. Any ideas what might be causing this?

After posting this, I went back to edit that same sweep command, and when I did, I noticed the sweep location arrows moved. I clicked ok (without doing anything) and now the surface looks fine:

Just not sure why this happens, occasionally.

Hard to say for sure, but from that video, it looks like this is probably just a faceting issue (any time you have triangle-shaped weirdness, it is probably something wrong with the graphics mesh).  When the surface was in the bad state, it would have been interesting to see what a round trip through IGES or STEP produced
