Shape tab in Form

In cad, the it is not allowing me to choose a shape in form. How do I get the tab that lets me choose shapes back?

I apologize, but it's not clear to me what your concern is.  But, I do have a possible guess.  Are you referring to the set of tools in the Sculpt workspace?:

If so, and if you have your preferences set to the default, your design will be a history-based (parametric) design.  In a parametric design, the Sculpt workspace is modal.  You need to first create a "Form" feature in your timeline.  Once you have done that, you will be put into the Sculpt workspace, and all the tools will be available.  To exit, choose "Finish Form" from the right side of the toolbar:
