Properly setting up a Ball Joint

I've created an assembly to learn about this joint.
I have set limits in the all axes of ball motion so that the cylinder-attached-to-the-ball should not contact the ball's housing.
I set limits to +/- 30 deg. which should avoid contact - but when I animate it does contact the ball housing..
2) When I enable a 'contact set' set to 'all bodies' it seems to recognize this BUT moves in a jerky motion (I don't know if this is normal, is it?).
3) I've also tried to set pitch to up and down, and yaw to left and right. And roll to be about the cylinder-attached -off-the-ball.
But to get it to work I have to make roll to move left-right and set pitch to roll the arm. I don't understand why.
I have tried rebuilding the joint. I used a custom setting when trying to define the ball joint, but the software won't let me set the y axis as the rotation axis for yaw.
