Joining / Filling between two dissimilar pipes

I am creating a wye ("Y") junction pipe and am stuck on to to join the pieces together. Loft would seem to be the right tool, but I can't seem to be able to figure out how best to get it to work (tried for hours).

Has to be a simple solution for such a common shape.

Any advice?

A Y-branch of done right is not a simple shape 😉
