Extrude doest not extrude all

So I created my sketch, but when I try to extrude something weird happens, when in the extrude mode everythings works as expected but when I actually 'commit' the extrude one 'tab' dissapears (the top middle one, see images below) Any idea on how to fix this ?

Does the missing tab move if you change the order you select in?

Nope doesn't change at all, I've just noticed it does extrude when I don't select the tab opposite to it.
