Design of pieces

Hey i need to make a piece and i dont know how. For now i only have the round piece and i need to make the extension that we see but only in one piece, the circle piece is in the post in .f3z.

If you know how to do that would help me a lot. (Sorry if i make somes mistakes my englis isn't good)

Does the small hole get aligned with the hole in the frame, or is there an adjusting rod between the two holes?  Is there another way to adjust the drive chain?
