In this sketch a rectangle was created, then a line offset down from the top edge. A line was added 2.5" above the origin:
Adding a dimension between the endpoints of the offset line and the added line creates a normal dimension, but both lines are already constrained horizontally as well as vertically dimensioned. Expected result is a driven dimension.
Note: If the dimension as created between the lines, not the endpoints, then a driven dimension will be created.

I think that isn’t a constrained bug, but yet another indication that there is something inherently flawed in how offsets are implemented.

I tend to agree with your statement. Offset is an expensive and not always reliable function. Yes, it is helpful in many circumstances, but it should be utilized as a last resort.. which is not the case in the example presented by Mr. ETFrench.
There are some siblings of the standard offset. Some are swimming in the Pacific Indian Oceans now, but if? I catch them,... I will 'show' them on the Forum.
