I can get the model now.

As far as I know, the existing form features cannot be combined into a single one.

But in this design, it looks there're no dependencies between the three form features. 

So maybe you could just remove the 3 existing forms and then create a new form that patches the 3 areas.

In fact, I don't exactly know the reason you want to combine those forms into a single. 

Perhaps you could give me more information about your intent?
