In the email it said:
"Thanks for submitting your documentation. Unfortunately, we’ve been unable to confirm your eligibility for educational access to Autodesk products. During the review process we found: The last name in the document does not match the last name on the verification form."
But the name on the document is the correct name.
What should I do? I only get 3 attempts at submitting a document, and I don't want to waste it. Any help would be deeply grateful, thank you.

Welcome to the Autodesk Students & Educators Community!
I'd recommend checking your image is clear and that there aren't any artifacts/glare distorting your image. If your image still appears to be fine, I'd recommend using the templates we have for both students and faculty, which should be accepted when filled with the required details.

Hey @LeoWarren-EDU, no wouldn't be the problem. The document I used is a very clear PDF, it should be fully legible.
I only have 3 efforts to submit, and I've already submitted 2, I don't really want to waste my last attempt. I would rather speak directly with Fusion for support.
