Different rotations for different annotation scales

Is it possible to have a dynamic block at different rotations/orientations depending on the annotative scale applied?  I have a block I use in civil drawings to show entry points into a site and I'd like to change it's orientation when in detailed views of a site (e.g. at smaller scales it would point to the inlet on a lake but at a larger scale it would be oriented to point towards the boat launch at that inlet).

I think it is possible. But before spending time on it, can you give an example in picture showing what you need. you can draw simple sketch with hand.
