Unresponsive Custom Flyouts on Toolbars

I'm attempting to create a flyout menu on a custom tool bar. I believe I have followed all the instructions here at the Autodesk help page.

 

it creates the desired Icon / button on my tool bar, and even displays an arrow in the lower right hand corner of it. But it only activates the first command in my flyout menu and does not expand to show the other commands.

 

Have any idea where I'm going wrong?

You will need to do a "long click" on it.

 

A quick click on the flyout only runs to top/last command.

 

Instead, hold down the mouse button for about 0.387 seconds and the toolbar will fly out.
