Pipe Plan Label That Flips

I want a pipe label for plan view that is a constant offset from the pipe outer diameter no matter the pipe size or drawing scale and also works when flipped. I can't seem to get this to work, even with expressions. Does anyone have any idea how this can be achieved?

 

If I use a standard Y offset, the pipe labels work for smaller pipe sizes but not larger ones. If we change that Y offset to work for larger pipes, it looks wonky for the smaller pipes. If we base the anchor point off of the top or bottom of pipe it will work in one direction but if we have to flip the label or the pipe is drawn in another direction the label doesn't work.

I've been using an expression to supply the Y offset value for the label.  The following is what I use in imperial drawings.
