If you are using feature line as alignment check the elevation of it, maybe it is correct at start and end but at the middle the level is 0, If so use the command set grade between two points and it will fix the error .

Does this happen at the start/end of a region for a baseline? If so, are you using a profile/feature line for targeting? C3D has trouble know what to do if you use a profile/feature line that starts exactly at the same station as your region. The easiest solution is to stretch your profile/feature line to start slightly before/after the region.
