Intersection Lane Grading

I'm using intersections for the first time and have 3 that are working well, except for one quadrant, shown in the attached image. I can't figure out why the grading here has gone awry. You can see it dips down to a low point three feet below where it should be. Any idea what I'm missing?

Its' always good to plot the profile of the curb return to see exactly what is going on and confirm the design intent.

Confirm the alignment is in the correct x-y position and then make sure z is correct in profile.

 

If the alignment & profile look good I would check the the intersection targets and the stations for each region forming the intersection.

Thanks for the tip. I removed a couple of PVI's from the profile (see image) and now the grading looks normal.
