Most likely what Joe said, try weeding it out a bit. Similar errors would happen....well, imagine a strong horizontal curve and a long grading (longer than the radius), grading obeys rules if it can, and it all gets crossed in and messed up, get some holes and whatnot 😉
just clear it up a bit and re-do.

HOWEVER, if I delete some points, such as PI points, the curve won't be smooth. It may lose accuracy.

If you are grading from a feature line with elevations extracted from a surface more then likely you have morethen you need. Additionally, the horizontal geometry to the feature line - what is it? Was it created from some sqiggly line or is it a true engineered line with curves? If your grading on the inside of a curve the geometry of the curve is important.
Gradings only know that you want to go so far and such and such.... we have to determine the design viability by verifying the geometry works with the request to the grading.
post your drawing. otherwise its all conjecture

you should post the file, and tell us what's your aim 🙂
Also, sometimes it works (for someone) to draw a manual feature line on grading hole ends, in a manner that would CLOSE the hole. Then - grading infill.
It's better practise to go about the origin geometry and rules, but sometimes this is good enough.
