Dimension symbol mystery

Does any one know what the yellow square with the exclamation symbol means next to dimensions.

We are all of a sudden getting this symbol and its confusing the engineers when they look at the actual cad file. 

What is it and how do I turn this off.

I have never had this problem before.

I have just discovered that when the above symbol appears a can not edit the text within the note or dimension.

In order to make a correction I have to delete the note and add it back in. And yes I get the yellow square symbol again.

Thank you. For some reason the "Annotation Monitor" (Bottom Right Corner) had been turn on.
