Adding Commands to toolbar creates invalid CUI - can't create custom toolbar

For three hours I have attempted to create a custom toolbar
or add a command to an existing toolbar.
example. I trimed down the Zoom toolbar, and added
Zoom realtime, Apply and all ok.
I attempt to add the Pan Realtime to the Zoom toolbar and
it will go there and the toolbar will appear, but when I come
back into CUI with CUI command, it says invalid CUI..
Any suggestion or ideas as to why the CUI file become corrupt
or what I can do to fix this?
