Error when smoothing surface mesh

Can anybody suggest what should be done to prevent this error. I am using manual mesh sizing.

Well, the immediate fix would be to use a larger size - or to run your cad model through a geometry validation and make sure that there are no errors. What mesh size are you using? I haven't seen this error in the wild yet.
