How to create a site form for the BIM 360 Docs

Hello all,

I purchased the BIM 360 Docs licence, Then i receive the e-mail from Autodesk ,they asking me to provide the site form for the BIM 360 Docs.

How do i create the site form ? And what are the steps ?
