I WANNA KNOW HOW TO ROTATE A CUSTOM ACTUATOR(BLOCK BASED) INDEPENDENTLY FROM VALVE(STANDARD VALVE)

No this function is not available in Plant 3D.
The only solution is to create 2 valves and connect the actuators to the valve body once with 0 degrees and once with 90 degrees

Is it possible to create block based valve and attach an out of the box actuator ? Or even a custom actuator so it could be rotated independently ?

No, it is not possible to turn the actuator separately from the valve body

You can do this by having additional actuator of the valve. I have tried to use this and it solve the problem somehow. This can take additional work.
