Line Gap Not Generating

I'm having a persistent issue of line gap going away. If I draw the lines first and separately from the equipment, it generates correctly as seen in screen shot below

But if I were to draw the lines from the equipment or connect the lines to the equipment the gap goes away and I start to run into other anomalies in the line behavior.  Examples below:

After shifting the right vertical line, the horizontal line becomes segmented without a grip point and the line is no longer level.

I've also placed Inline Assets and the line behind is not blocked out.

I have started a new drawing and placed new cells/lines and seen this issue persist.

Additionally, I've been seeing a much higher incidence of ACAD Error codes causing the program to shut down and sending error report to AutoDesk.
