I am struggling to get this drawing to print in black and white. I believe I have everything in Index colors and my visual style is 2D wireframe. From the forums and discussions I have seen. The Monochrome STB would have every index color in the form view. Mine is blank, but I am unsure how to fix this, if this is even the issue.

all object plotstyles are set to bylayer, all layer have plotstyle "Normal" *
Plotstyle "Normal" is set to print in colors..
*except 4 of them, this is not ok: Corrupt data!
open your file by RECOVER or run AUDIT yes.
If you like to print in black, set all layer plotstyles to Style1, what is set to print black

For .stb depending drawings, the color of layers or objects are completely uninteresting,
it's all about the assigned plot style.
If you read anything about Ondexcolor and plotting, then it was about .ctb

You are using STB plot styles: these are not what you read about, those are CTB plot styles.
A DWG file cannot be both all at once.
If you are committed to STB plot style tables, instructions are not in the excellent post above on how to do that.
If you don't care to use STB or have only ever used CTB plot style tables, see the attached converted DWG to do just that with the stock monochrome.CTB (not STB) plot style table
That's not what the Monochrome STB looks like in the Plot Style Table Editor as it has 9 Plot Styles just for grayscales alone from 10% to 90% so something is wrong with the one you have open.
