I have a drawing template with about ten page setups in it. If I open this on one PC with AutoCAD 2023 or 2024, this is not a problem and I can see all these page setups in the page setup manager.
I am currently working temporarily on a PC with AutoCAD 2021. If I now open a new drawing and select this template here, no page setup is visible here. I only see the name of a page setup I created behind *layout1*", *layout1*(A1) in this case.
If I open the template directly from Windows Explorer, the page setups are also not visible.
If I now save my template on my one PC as a 2018 drawing (not as a template), and then open it in 2021, all page setups have disappeared here too.
Does anyone have an idea why the page setups disappear?

Either your AutoCAD2021 is opening the same named file (but it is not the actual new 'same name' file with all the pagesetups), or your AutoCAD2021 is completely damaged and needs a full removal and re-install plus all the updates.
I suspect the former being the likely issue: share your DWT that woks everywhere but in 2021 here an we can confirm or find other issues if they exist on your behalf.

I have added the drawing template to this reply for you.
Below you can see all created page setups in this template. There are also all layer filters in the layer properties manager. These were also gone in AutoCAD 2021.
That these filters are not visible in 2021 is not a big problem. With 1 lisp command they are all created again.
ps. the template I made is saved in the default AutoCAD set "drawing template file location". When making a new drawing I always select this and not a template from another file location.

I will not ask why you edited standards (acad.dwt), instead of create your own stuff (individual names),
you don't have benefits by using standard (names) for your custom stuff, it only increases the possibility of causing unnecessary problems.

Any difference if you run in AutoCAD 2021 the PAGESETUP command, select the Import button and then locate & select the template file?
Note: In the Select Page Setup from File window make sure at the bottom the “Files of Type:” shows Template (*.dwt).
