Multileader block with attached attribute annotation

Is there a way to create Multileaders with attached attributes such as in the attached? The attached shows a Multileader with a circle and leader and also Mtext with a line.  I'd like to have all in a block where you can edit the Multileader anno, the external anno and the line will extend or shorten in relation to the text.

This would be a block of yours, all of it (what you need to behave like "mtext" can be a Multi-Line Attribute instead)

You would add that block here in MLEADERSTYLES command

No, you cannot slip in a block inside the MTEXT editor (which is also used in MLEADER). Sorry. That's not a thing in AutoCAD to date.
