Is there a way to determine where a block in a drawing was inserted from?

XREF, yes. Block, no way.

well, nothing on the CAD side -- but you can search the various drives for the blockname.dwg.  That won't guarantee that the file you find was what was used, but may be worth a shot.

A sometimes answer:  If  [admittedly a pretty big if] no Block has been inserted from an external file in a different location since that one was, then that location will be what comes up to choose in when you pick Browse... in the Insert dialog box.  But that means no other external-file-location was the source of a Block inserted in any drawing, not just the current one, so it may be of no real use to you except in pretty particular circumstances.
