Basically, what you are saying is that you can load the graphics into the block creator, but any other command (like adding an attribute) returns a response of 'NIL'.
Have you tried to create any other blocks since you started having this problem?
I just tried using 2014 (will try 2016 over the weekend) and had no problems with adding the attributes or saving the block.
Note that I tried the old Black box builder as well as the standard block builder.
One thing I did find is that you have a lot of residual gragphics an blocks on 3 layers that are turned off.
Turn those layers on, explode the graphics that show up and then delete them.
Also detach and remove any XREF's you have attached as they may be causing problems as well
Next you need to run the OVERKILL command to reduce the number of duplicated graphics.
Finally run the PURGE command to remove any extraneous layers.
All this will cause the block created at the end of the process to be a lot smaller in file size as well as not cluttering up any dwgs you may use it on.
After all that, try to create the block again, and if it doesn't work
Give it a try using the black box builder as it seems to be more robust and with less chance of an problem.
The command to start it is WD_SYM_BUILD.
If all else fail, try copying the graphics to a new blank dwg and then creating the block.
