Basic question modeling a square shape in a cylinder

This driving me kind of nuts. Should be basic stuff! 😐
But what technique are you using to model a square shape into a cylinder object for instance? Where the edges are chamfered and turns out smooth and square when applying the turbosmooth i.e modifier? Not boolean which messes the geometry up. I've tried to use the inset to make some supporting edges i.e. But there're always some artifacts somewhere. Below an artifact can be seen in the corner.
These many polygons just to have a square with some chamfered edges cant be right?
