Improve Gizmo: add extrusion handle & overall transform mode

The gizmo could be improved by adding an additional handle, which is used to perform extrusions on vertices, edges and faces.

(This is Rhino).

It would also be beneficial to have an overall transform mode in which scale, movement and rotation can all be controlled at once.

I wonder if all these great gizmo related suggestions couldnt be (also) grouped in a bigger idea ! 

Max definitely needs a better interaction with its tools and the gizmos (along with the tools cursors, visual cues etc) its the first step in this direction. Besides updating the functionalities of its gizmos and adding more visual cues I  might add the update of the graphics of all of these. Some are very pixelated or just ugly or dont give enough info. Cinema 4d and now Blender start to have  (or already have) a very cool and modern way to interact with its tools (and they also pay attention to details).
