I sort of have an idea why, though not sure about the idea how to fix it. Usually, the ampersand (&) precedes a menuitem letter that works as a shortcut (and that letter is often rendered with an underline to show that). In html, that's escaped as & and that's also how they are saved in the .mnux file:

customTitle="&Create"

Could it be possible that they were double-escaped, resulting in '&Create' ? Could you check the .mnux file? Or replace it with the same file from another working install?

Hello @Anonymous,

@Swordslayer has an excellent post here and I wanted to ask you, did you recently install something which caused this to occur?  You can always reset your user settings and that should remake all your menus and interface items.  Can you please try that for me?
