3ds Max 2016 Right Click Error

Everytime i right click on any viewport i get the script screen and error messages as seen below both the screencast and screenshots. What could be the problem?

Rail Clone and Forest Pack plugins not installed in my system and i get this message in the 3ds Max 2016. Thanks for the responses.

2 things i can advise..

1 - reset your quad menu on its default state

2 - and or delete that particular scripts on your folder

It was already a fresh setup. Issue happened on the first run. Anyway i tried your advice and resetting the UI has worked.
