I believe this happens with ductwork as well.
Let's say I model a pipe at 8'-0". Then I tag the pipe. If I relocate the pipe in the X or Y plane, the tag moves with the pipe accordingly. However if I change the middle offset of the pipe, the pipe tag relocates parallel to the pipe. The distance that the tag moves seems to proportional to the change in the offset of said pipe.
This can be troubling when many times we would relocate a pipe in our modeling view, but the tags would move away in the Annotating view. Then we sometimes won't notice the problem until after we send our documents out.

Yes, this has been happening for many years. I never noticed a correlation between the offset change and the location change but I've "lost" many tags over the years.
Since there is no way to select the tag by selecting the element like you can the other way around, the best advice I can give is to remember to delete the tag before changing the offset and then re-tag.

Another option would be to turn on the leaders and set them as attached.
