Deleting a Header from the Insert Sheet Lists Table

I have successfully created a table style named Index that uses only a Title and Data and leaves off any Header.

However, when I try to use this table as a style for createing the Insert Sheet Lists Table from SSM, the Header reaapears.

Please help if you know the answer.

I HAVE THE SAME PROBLEM WITH AUTOCAD 2014.
