Custom Message Box

Can anybody point me to sample code to create a custom message box that has
a "Dont show this message box again" check box included

You just create a new form, and place the controls you desire. A couple of
labels and a check box. Store the check box status in the registry or
external file or in the drawing's dictionaries, what ever makes sense for
the application.
