Files/Items vulnerable to editing when Change Order is in Check & Review states

When an Item is controlled by a Change Order that is in the Check or Review states, the files remain editable & the Item can be updated. An example is we have Unanimous Approval Required routing enacted & have four Approvers. If one of the Approves approves the change order, the files can be checked out, edited & the Item updated. This allows one Reviewer to have approved a different version than the other three Approvers. In theory, all four Approvers could approve different versions.

During my exploration of this issue, when I select the drawing file on the change order, for example the Controlled by Change Order property shows False. When in the Item window, the Controlled by Change Order property shows True. While this does make some sense it seems there is significant vulnerability to the files, item & the integrity of the change order process.

I realize you are not supposed to do things like this but people will be people & the Vault should prevent this type of action.

Is there something I'm missing?

Are you using Item security on the file when it is in the Review state?

According to your settings, no one can checkout or edit a file in that Item state.

If you look at the security associated to the Item in this state, what does it show?

Are you using File Lifecycle states on the files at the same time you are using Item Lifecycle states with security?
