Add Approver and comments to CR/CN report

Adding approver and approver comments to the CR/CN report would help out a lot when sending out the reports. The jasper reports has a cr_assignees dataset but that shows the users currently assigned a task which after the CR is completed shows up blank.

Ideally what I would want in the CR/CN report is what is shown under the Tasks tab of the CR. Completed by, completed date, and comments.
