How to refine mesh locally and avoid incohesion due to geometry discretisation

I am modelling a cylindrical shell structure and I want to have control over the mesh density in different areas.
Ideally I want to control the density in very localized areas (both in elevation and plan) but I can settle for refining the mesh density at different levels only.
The issue I have is that there doesn't seem to be a way to avoid incoherence. Take a simplified example (screenshot attached):
- I want to use 100mm mesh size in the top of the shell;
- I want to use 1m mesh size in the bottom;
- I have set up these two regions with a small region I want to use for transition in the middle.
No matter what I do, I can't achieve any sort of coherent transition in that region. Any suggestions?
