Instrument and Control schedule

Hello, I am working on a model which has a long list of equipment that speaks to each other.  My schedule needs to have a number of things shown similar to the schedule below:

1. equipment start point (duct from/location description)

2. equipment termination point (duct to/location description)

3. conduit size (duct size)

4. content inside each duct (cable type)

 

The cable types are not only low voltage signal, but there are chemical lines and air lines which will be connected and/or triggered sending this media to other equipment.

 

Please let me know if you have any ideas or have done this type of schedule and the depth of parameters needed on each family,

I am working with Keith and we are still hopeful there is a smart BIMy way to accomplish this task.  Since his original post we have developed our thoughts and knowledge a little more, so I'll take a stab at explaining what we are trying to do in the simplest terms I can.

 

We would like to schedule conduit runs where the panels each conduit run is connected to on each end is scheduled as well.  Our hope is that Revit will allow us to just run a conduit from one connector to another and a schedule will report what the conduit is connected to.  Here is a sketch of that:
