How do I add a second row to a revision schedule?

The revision schedule i'm using needs to be two rows and I can't figure out how to add a second row. I made it work by adding two different revision schedules, the second one being for the description. Then when i add another revision to my sheet, it doesn't work like i was hoping. If i could make a single revision schedule with two lines it would work great. Any ideas outside of dummy text on every sheet? This is a clients border that i need to use.

edit your title block, select the Revision schedule, select Edit for the Appearance option, se the Height to User Defined or Variable.

I'm trying to get a second row in the current rev to show the description. From what I can see, I can only add another column. What they want though is .....issued to/rev#/date/by on top then the description located directly under it like my second picture shows. Then when I add another revision, I need it to move down an additional line (2 not 1) so it still shows the next revision descriptions for both revisions. The third picture shows how its actually working, its not showing the descriptions.
