Elevation Tag Questions on Duct & Duct Fittings

I have two questions about Elevation tags

 

1) How do I get Elevation tags to acknowledge Insulation on Duct?  In the attached screenshot, you will see that I have some duct.  The Elevation tag shows the bottom at 102"; however, this is to the bottom of the duct metal.  The duct has 3 inches of insulation, so it should actually be showing T: 117" / B: 99" - how can I get Revit to display this properly?

 

2) How can I get elevation tags for Duct Fitting type Families?

Have you tried Spot Elevation?
