for Toposurface the only way is Building Pad - that allows only voids from up to bottom - opened at the upper part...
If you want to make somethig like a tunel or a horizontal or sloped mine shaft , then you'll have to use either a Model in Place terrain or a Mass model in place...

You are correct that the building pad is the only way to create a void in a topo surface, but you are incorrect about a model in place or a mass model to create a horizontal or cave opening.
As I have noted in many previous posts, a graded region can be placed over a building pad void and it will follow the original topography. Leaving a portion of the graded region open will create a tunnel, cave, or horizontal opening into the building pad void.

In my opinion the Toposurface function is not very developed. Anyway Revit is not a program for ground works and I suppose AutoCAD Civil 3D or other similar programs are more suitable..
About your method looks to be ok ,but I wonder how do you solve the tunnel profiles or underground vaults like these:
What I said ,in my previous post ,is that I prefere for small terrain areas with undergound buildings with complex shape ( vaults or arch profiled) to model myself the terrain using Mass Elements or Component Model In Place .
