You can edit the type and change the Leader Arrow Head (use the Arrow Open 90 degrees) and set the Symbol to <none> ....

Note:

  1. Change/Edit Leaders under Manage >> Additional Settings >> Annotations >>> Arrow Heads (But somewhat limited configuration and you have to work with the built in Options)
  2. The Symbol is a family you can edit/customize an existing or make new

Edit: Some OOTB  project templates have that type (No symbol) already setup...Select the Spot Elevation and check under the type selector/drop down menu other wise duplicate type and create one as described above

Ah ok now I get it you want a gap between the Arrow head and the Building element on which you placed the spot elevation... there is a workaround or a trick  to set up something similar...Below

  1. I created a custom symbol by editing the existing cross hair and saving new (Left window)
    • Model the arrow head away from the 2 intersecting reference planes
    • add the symbolic lines representing the Shoulder and Leader
  2. In the project duplicate a spot elevation type and
    • Either set the Leader in to None
    • OR switch off the OOTB Leader and Shoulder of the spot elevation after placing it
  3. Add the symbol from step 1 to the Spot elevation
  4. Place and drag it outside the wall

Yes this is what I was looking for, I though may be there is already build in Revit such type of arrow, now I understand that I need to make one myself.

Thanks for step by step tutorial.
