After getting well into a model I realized that True North was about 111 degrees off. I set the True North to be accurate and then rotated project north by 90 degrees. I then had to rotate the plan views placed on the sheets.
The model is now just slightly off the North-South axis so that when I adjust walls I get an error that walls are off-axis. They are off by 1/64" or less. I tried rotating project north via aligning a grid line/wall plane to N-S and that did not work. I tried adjusting the project units and then rotating. So far nothing works....
Disclaimer: This is my first Revit model and I now understand that setting the site/north is the first thing I should have done.
"Walls are off-axis"? You sure it didn't say "Element is slightly off axis and may cause inaccuracies"?
Project North/True North and Sheet Rotation are unrelated.
Answer for me this question: How far from the Internal Origin is your Model?
Why don't you just rotate True North 21 degree and leave Project North and the views on sheets alone?
Is it possible to draw a straight 90deg line north/south, rotate it off the 1/64th and then rotate project north? I know it sounds like it'll accomplish the same goal but Revit can be tricky sometimes and the way you do it can have an effect on the outcome.

Sounds like you added elements (the wall in this case) to a view that had a slightly rotated crop boundary. You say you tried to aligned wall to grid line. Is the grid line off-axis as well? If so, try placing a new grid line in a new view and then aligning the wall (as well as the off-axis gird line) to that new grid line.
The project origin is in the center of the model, 15' from the main entrance. It is set to 0,0,0, see attached imaged. If you mean how far the model is off by angle it would be immeasurably small. Less than a degree. Anytime I try to measure I get 1/64".
Turns out the issue was limited to one plan view, which I had rotated 90 to fit on a sheet. Must've just snapped slightly off-axis. All other plans were alright (walls and gridlines were on axis). Having just messed with the Norths I assumed the worst. A beginner's mistake! Thank you all very much for weighing in.

Hopefully you didn't place too many elements in that rotated view. Have you inspected the Warnings in the Project? It's a good idea to keep an eye on Warnings as you model. Resolve them as you progress - while they are resolvable. Don't let them build up.
