I've been struggling to carve out an underground tunnel, clear of dirt/ backfill.
The building pad command is a "virtual" vertical void geometry, it will only clear the "dirt" out vertically from the top of Toposerface to the set bottom of the building pad.
Tunnels/ tubes do require this application BUT horizontally, the question is how ?
Any help/ tips would be highly appreciated,

Unfortunately 'tunneling' horizontally in Revit with 'fill' around the 'hole is currently not possible.
you can use buidling pad, but the is flat and while you can slope it it may not suffice. The only way is to create the structure you need and use a masking region to 'cut' out the area you need, alternatively use a building pad and use fill pattern to 'fill' over the 'tunnel'.

You might try adding a floor above the tunnel of required thickness with earth properties. This can be made variable to accommodate the terrain above.
